I/We acknowledge that SKP Park of the Sierras has presented me/us with the option to execute POS 36, the “Payable on Death (POD) Beneficiary Designation Form” which will provide instructions to our Park Manager and Treasurer regarding the refund of my/our Membership fee upon the death of the last surviving member.
