Plan Service Area definition

Plan Service Area means a geographical area in which a Network of preferred providers is offered and available, and is used to determine eligibility for managed health care benefits under Member’s Plan.

Examples of Plan Service Area in a sentence

  • Inasmuch as this Contract replaces a Contract between Blue Shield and the Employer, each individual in the employ of the Employer on the effective date of this Contract who was a Subscriber of Blue Shield by virtue of the Employer's previous Contract on the date immediately preceding the effective date of this Contract, who lives and/or works in the Plan Service Area is eligible on the effective date of this Contract.

  • The geographic area served by this Plan is defined as the Plan Service Area.

  • The directories will list names, locations, and other information specific to those physicians, hospitals, and other health care providers and facilities in the Participant’s designated Plan Service Area.

  • The appropriate foreign languages comprise all languages in the Health Plan Service Area spoken by approximately five percent (5%) or more of the total population.

  • Subscribers and Dependents must live or work within the prescribed Plan Service Area to enroll in this Plan and to maintain eligibility in this Plan.

  • In addition, Health Plan may, subject to government approval, amend this Agreement at any time by giving 45 days prior written notice to Group in order to (a) comply with applicable law, (b) reduce or expand Health Plan Service Area, or (c) increase any benefits of any Medicare product approved by HCFA, if applicable to this Agreement.

  • Other Amendments Health Plan may amend this Agreement at any time by giving written notice to Group, in order to (a) address any law or regulatory requirement; (b) reduce or expand the Health Plan Service Area; or (c) increase any benefits of any Medicare product approved by the Centers for Medicare and Medicaid Services (CMS), if applicable to this Agreement.

  • Coverage will stay in effect as long as you and your Covered Dependents continue to meet the eligibility requirements, live in the AvMed Engage Plan Service Area, and pay your Premiums on time.

  • Coverage will stay in effect as long as you and your Covered Dependents continue to meet the eligibility requirements, live in the AvMed Empower Plan Service Area, and pay your Premiums on time.

  • Within the Empower Plan Service Area, Members are entitled to receive Covered Benefits and Services from AvMed Empower In-Network (Participating) Tier A or Tier B Providers, or from Out- of-Network (Non-Participating) Providers, as described in this Contract.
