Plan Physician definition

Plan Physician means a Physician who has an independent contractor agreement with SHL to provide certain Covered Services to Insureds. A Plan Provider’s agreement with SHL may terminate, and an Insured will be required to select another Plan Provider.
Plan Physician means any doctor of medicine, osteopathy, podiatry or dental surgery licensed by the State of California who has agreed to provide Professional Services to Members, either under an agreement with Plan or as a member of a PMG, and who is included in the selected Plan Network. Plan Physicians are listed in the Provider Directory.

More Definitions of Plan Physician

Plan Physician will mean a Physician who is under contract with Health Plan, either directly or through a contract with a medical group, IPA or other organization, to provide professional medical services to Enrollees.

A specialist physician may provide specialized treatment for the compensable injury or illness and give advice or an opinion regarding the treatment being rendered, or considered, for a patient’s compensable injury.

  • Consulting physician means a physician who is qualified by specialty or experience to make a professional diagnosis and prognosis regarding the patient's disease.

The outcome of this service is sustained paid employment at or above the minimum wage in an integrated setting in the general workforce in a job that meets personal and career goals. Supported employment services can be provided through many different service models.
