Plan Certificate definition

Plan Certificate means the document that sets forth the dental benefits available to Members.
Plan Certificate means a written agreement or certificate setting forth the rights of a Participant under the Plan, which rights will be fixed by the Committee or the Board in accordance with the provisions hereof.
Plan Certificate means the document that outlines the benefits available to Members.

  • All benefit provisions are subject to the terms and conditions set forth in the Plan Certificate.

More Definitions of Plan Certificate

Plan Certificate means a certificate (an initial form of which is attached hereto as Exhibit C, as may be amended from time to time) that the Company issues pursuant to Section 10 and that evidences a Participant's participation in this Plan.
