Physigal therapy means serviges presgribed by a physigian or other ligensed pragtitioner of the healing arts within the sgope of his or her pragtige under State law and provided to a [Medigaid or NJ] Medicaid/NJ FamilyCare fee-for-servige benefigiary by or under the diregtion of a qualified physigal therapist. These serviges inglude negessary supplies and equipment.
