Physician Lead means a doctor of medicine, screened by the Contractor to ensure the physician is currently licensed to practice medicine in the state of Washington, without restrictions, disciplinary action, or open licensure investigation in the United States.

  • Further, existing groups must submit the enclosed document (Family Health Group Governance Certificate) confirming the completion of a written governance agreement and appointment of a Physician Lead and Associate Lead to the MOHLTC no later than February 1, 2015.
