PHB Nurse definition
PHB Nurse means the person nominated by the CCG to monitor and review the Direct Payments in accordance with Paragraph 2.2 of Schedule 1. The PHB Nurse will be the Patient’s or Representative’s single point of contact for all PHB related queries.
PHB Nurse means the person nominated by the ICB to monitor and review the making of Personal Health Budget Payments in accordance with Paragraph 2.2 of Schedule 1.

The Patient or their Representative (where applicable) agrees to notify any change of address and/or GP to the PHB Nurse.
In all cases the PHB Nurse will first attempt to resolve the matter through dialogue with the Patient and/or their Representative and/or the Third Party.
