PCPT definition

PCPT means Person Centered Planning Team. A PCPT is comprised of individuals including the Person, the Person’s legal guardian (if applicable), the Person’s parents and other family members, the Person’s SC, the Contractor, and any other individuals the Person would like to participate on the PCPT. The PCPT assists the Person to create their PCSP (see definition below) and identify necessary services and supports based on the Person’s desired life and support needs.
PCPT means The Presbyterian Church Property Trustees.

Examples of PCPT in a sentence

  • The CHC-MCO must include the PCPT approach as part of the service planning and Service Coordination processes for Participants who require LTSS.

  • The CHC-MCO must develop a PCPT policy for PCSP development and implementation for Participants who require LTSS.

  • The CHC-MCO must annually submit and obtain Department approval of its PCPT policy prior to the expiration date of the previously approved policy.

  • Workforce innovation that improves the recruitment, retention, and skills of direct care workers, which may include but are not limited to direct or enhanced payment and other incentives to Providers, Participant-Directed employers, and direct care workers for education, training, and other initiatives designed to enable direct care workers to become a more functional member of the PCPT.

  • The PCPT approach must comply with the PCPT requirements of 42 C.F.R. § 441.301(c)(1) through (3) and of this Agreement.

  • The CHC-MCO may include the PCPT approach as part of the overall care coordination approach for Participants who do not require LTSS.

  • The CHC-MCO PCPT approach must be person-centered and must take into account all goals and requirements of CHC.

  • PCSPs must be developed by the Service Coordinator, the Participant, the Participant’s representative, as appropriate, and the Participant’s PCPT.

  • For participant’s receiving LTSS, the orientation must also include the following topics: • The role of the Service Coordinator, • The role of the PCPT, • PCSPs and the service planning process, • Participant Self-Directed models (for Participants receiving HCBS), • The role of Service Coordination Unit and how to contact it directly, if necessary.

  • The CHC-MCO PCPT approach must be person-centered and must consider all goals and requirements of CHC.
