Patient User(s) definition

Patient User(s) means all Users who are Clinic patients as well as any Designated Proxy of these patients.

  • Customer is solely responsible for inviting individuals to become Patient Users; properly identity proofing and authenticating such individuals before providing them with access to the Patient Portal; and providing technical support to Patient Users with respect to their use of the Patient Portal.

  • Patient Users: In order to access certain features of the Site and Services, you must register to create an Account and become a “Patient User.” During the registration process, you will be required to provide certain information and you will establish a username or email and a Password.

  • Once you have access to the Portal, you can click on the "Help" button in the upper right-hand corner of the webpage to read the Patient Users Guide.

  • Customer shall provide its Patient Users with access to the Patient Portal.

  • Customer understands and agrees that Patient Users will be required to register to be able to access the Patient Portal.

  • Patient Users are solely responsible for making decisions on the choice of these providers for their personal health needs.

  • Customer shall promptly notify AZCOMP of any unauthorized use or infringement of the Patient Portal or any other e-MDs’ intellectual property by Patient Users or others, of which Customer becomes aware.

  • Any disputes between Healthcare Providers and the Patient Users will be dealt with by Healthcare Providers directly.

  • Customer is responsible for defining what clinical content will be available to Patient Users through the Patient Portal and making such content available.

  • During the registration process or anytime thereafter, Patient Users may be asked to accept terms of use and a privacy policy related to their access to the Patient Portal (collectively, the “Patient Portal Terms”).
