PATIENT CONTACTS means any admission, consultation, procedure, physical response to emergency call, evaluation, treatment, or service performed in any provider-based facility operated by the Hospital or affiliate, including outpatient facilities (provider-based means facilities that bill under the Hospital’s Medicare certification number). Patient contacts do not include referrals for diagnostic or laboratory tests or x-rays.

  • Bona fide physician-patient relationship means a treatment or counseling relationship between a physician and patient in which all of the following are present:
