Paper Claim Form definition
Paper Claim Form means the Court-approved paper (not electronic) claim form, without material alteration from the sample paper claim form attached hereto as Exhibit 2, that a Settlement Class Member may submit to be considered for a Claim Payment under the Final Settlement.
