Cosmetic Services Services that are intended primarily to improve your appearance and that are not likely to result in significant improvement in physical function, except for Services covered under “Reconstructive Surgery” or “Cleft Lip, Cleft Palate or Both” in the “Benefits” section.
This exclusion does not apply to dental services for which Benefits are provided as described under Treatment of Cleft Lip or Palate or Both in Section 1: Covered Health Care Services.
