Ordering Physician definition
Ordering Physician means a person legally qualified and licensed to practice medicine and legally authorized to request genomics testing.

Institution’s/Facility’s medical staff or designee, shall ensure that service request is clearly and completely filled out providing contact person’s name and phone number, Patient’s/Youth’s name and CDCR/YA identification number and Ordering Physician.
