Examples of Optometric Services in a sentence

  • In consideration of the optometry services to be provided, State shall pay Contractor according to the following schedule: Contractor must bill all Optometric Services which include vision examination and fitting/dispensing eyeglasses to Conduent on an HCFA-1500 claim form.

  • Optometric Services Not covered for Members age 21 and older: routine refractive services and optical devices.

  • Genetic services means a genetic test, genetic counseling (including obtaining, interpreting, or assessing genetic information), or genetic education.

  • Home health aide services means the personal care and maintenance activities provided to individuals for the purpose of promoting normal standards of health and hygiene.
