Opt-Out Request Form definition
Opt-Out Request Form as used herein means a form that will be available for download on the Settlement Website, which can be used by Settlement Class Members to Request to Opt Out. Settlement Class Members are not required to use the Opt-Out Request Form in order to communicate their desire to opt out of the Settlement.
Opt-Out Request Form means the form attached hereto as Form 1-D, signed by a Rule 23 Class Member, which must be submitted in the manner and within the time frame set forth in the Class Notice.
