On Formulary means that all NDCs of CRESTOR are listed on Eligible Member’s Formulary, or if Eligible Member does not have a Formulary, Eligible Member shall adopt GPO’s Formulary, as a preferred brand name drug that has been approved by the U.S. Food and Drug Administration.
