The request will state that the comments must be received in writing and that the comments will be included in the Official Performance File of the Librarian being evaluated.
Where they agree with the content, they shall sign the letter and return it to the Chair and a copy of the Annual Review with both signatures will be placed in the Faculty Member’s Official Performance File.
A copy of the reviewer's report will be sent to the Faculty Member and the Chair and a copy of the report will be placed in the Faculty Member's Official Performance File together with a copy of the initial or revised Annual Review.

Nothing in this Article precludes a Member from including in her or his Official Performance File, on any subsequent consideration, any of these documents.
A Committee may not require a candidate to submit any material not in the candidate’s Official Performance File.
