  • You are solely responsible for the accuracy, quality, integrity, legality, reliability, appropriateness, and intellectual property ownership or right to use of all Customer Data, and OCHN shall not be responsible or liable for the deletion, correction, destruction, damage, loss or failure to store any Customer Data.

  • The Service Provider also agrees to immediately notify the purchaser of its services, OCHN, the Self-Determination Administrator, and the Financial Management Services entity of any felony or misdemeanor convictions or other events that could impact its ability to perform Behavioral Health and Intellectual and Developmental Disability Supports and Services.

  • Should OCHN decide to end this Agreement; OCHN will send prior written notice to the Individual/Legal Representative.
