These notes include dates, times, who attended, relevant conversations, observed interactions and activities, any incidents that occurred during the visit and the child/ren’s responses to the visits. These notes are strictly observation notes. MCCS staff do not make judgements or interpretation.

Examples of Observation Notes in a sentence

  • During the twice monthly meetings, the principal or supervisor shall use Appendix C-5 Drop-In Observation Notes and the written evaluation (Appendix C-8 or C-9) to identify progress made by the employee, if any, and to outline further steps needed to improve areas of deficiency.

  • At the conclusion of each conference where printed Observation Notes or the Observation Report forms have been used, both the employee and the evaluator will sign the form indicating that the form represents a report of the information communicated at the conference.

  • If a suggestion or recommendation for improvement is given in written form, it shall be sent utilizing district email, or the Informal Observation Notes Form, in such cases, the practitioner shall be given the opportunity for a conference with the facilitator/administrator, unless waived by the practitioner in writing.

  • Following each observation, or series of observations, the principal or supervisor shall promptly document the results of the observation in writing, and shall provide the employee with a copy thereof within three (3) days after such report is prepared (Appendix C-2 Observation Notes).

  • The evaluator will document the observation of teaching and will provide the Formal Observation Notes to the teacher 24-hours before the post-observation meeting.

  • For a Traditional Evaluation, there shall be at least one (1) observation, lasting twenty to fifty (20-50) minutes, which will be followed by a conference within ten (10) working days in which the evaluator and the evaluatee shall review and discuss the unit member Observation Notes.

  • The worksite observations shall be recorded on Appendix C-5 Drop-In Observation Notes with a copy provided to the employee within three (3) days.

Related to Observation Notes

  • Redemption Notes means unsecured subordinated promissory notes of the Trust having a maturity date to be determined at the time of issuance by the Trustees (provided that in no event shall the maturity date be set at a date subsequent to the first Business Day following the fifth anniversary of the date of issuance of such note), bearing interest from the date of issue at a market rate of interest determined at the time of issuance by the Trustees, payable for each month during the term on the 15th day of each subsequent month with all principal being due on maturity, such promissory notes to provide that the Trust shall at any time be allowed to prepay all or any part of the outstanding principal without notice or bonus;

  • Fixed Rate Notes Together, the Class A-2 Notes, the Class A-3 Notes, the Class A-4 Notes, the Class B Notes, the Class C Notes, the Class D Notes and the Class E Notes.

  • Convertible Notes means indebtedness of the Company that is optionally convertible into Capital Stock of the Company (and/or cash based on the value of such Capital Stock) and/or indebtedness of a Subsidiary of the Company that is optionally exchangeable for Capital Stock of the Company (and/or cash based on the value of such Capital Stock).

  • Subordinated Notes means the Initial Notes and the Exchange Notes and, more particularly, any Subordinated Note authenticated and delivered under this Indenture, including those Subordinated Notes issued or authenticated upon transfer, replacement or exchange.
