This policy defines the roles and responsibilities of the NSP provider, the ACS Close to Home Permanency Planning Specialist (PPS) and Community Support Specialist (CSS), and the DOE transition counselor and coordinator in developing and monitoring the youth’s educational plan while in placement, including transition to aftercare and completion of juvenile justice involvement.
