Network tier, definition
Network tier, means a set of network providers made available at the same cost-share level, within a tiered network. A network tier does not include providers accessible to enrollees through an out-of-network benefit.
Network tier, means the providers and facilities associated with a particular cost-sharing level in a network in which different providers and facilities are associated with different cost-sharing levels within a single network.
