You contact Us in advance of receiving any Covered Services, and We authorize You to go to an available Non-Network Provider for that Covered Service and We agree that the Network Cost Share will apply.
Under Indiana law, the Non-Network Provider can only collect from You the Network Cost Share amounts (Deductible, Copayment, and/or Coinsurance), if any, that You would be responsible to pay if the Covered Service had been provided by a Network Provider.
Because We have authorized the Network Cost Share amount to apply in this situation, You will be responsible for the Network Copayment of $25 and We will be responsible for the remaining $175 of the $200 Maximum Allowed Amount.
If You can show that You received inaccurate information from Anthem that a Provider was Network on a particular claim, then You will only be liable for Network Cost Share (i.e., Copayments, Deductibles, and/or Coinsurance) for that claim.
