The parties to this agreement are The University of South Dakota (USD) and Northeast Community College (NECC).

Individuals may request credit (a maximum of 6 credits) at NECC upon completion of this form and submission of their credentials for the purpose of authentication by a designated member of the Criminal Justice Program.
I understand that multiple returned payments and/or failure to comply with the terms of any payment plan or agreement I sign with NECC may prohibit my eligibility to register for future classes at the college.
The agreement applies to students who graduated from NECC in 2020 or later.
I authorize Northern Essex Community College and its agents and contractors to contact me at my current and any future cellular phone numbers, email addresses or wireless devices regarding my delinquent student account, any other debt I owe to NECC, or to receive general information from the college.
