County wishes to provide funding to City for implementation of the Housing First Model Program and City wishes to implement such program, as hereinafter described.
Participants whose TIN includes NPIs of a “Primary Care Practitioner” who bills “Qualified Evaluation and Management Services” (as both terms are defined by the Vermont Medicaid Next Generation Participation Agreement) may not participate in more than one Medicaid Next Generation Model Program, or any other Medicaid ACO-based payment reform program or with any other accountable care organization in which they attribute or align lives.

The program shall be based upon the nationally recognized San ▇▇▇▇ Model Program and shall be of a duration of no less than twelve (12) weeks.
The sole purpose of this Agreement is to define the financial relationship between County and City as it relates to funding of the Housing First Model Program.
City shall do, perform, and carry out implementation of the Housing First Model Program to serve individuals who meet the criteria for chronic homelessness and have a disability (as that term is defined by the United States Department of Housing and Urban Development) in a satisfactory and proper manner as determined by County and in conformance with the criteria outlined in Appendix B (Purpose and Outcomes), which is attached hereto and incorporated as if fully set forth herein.
