MEID means a 40 character mobile equipment identifier assigned to a Device by the manufacturer in place of an ESN.

• I represent that I have not altered the ESN or MEID of any Device submitted by me for the purpose of obtaining unauthorized use of telecommunications services, nor have I configured any Device to insert or modify telecommunication identifying information so that the Device may be used to obtain telecommunications service without authorization, nor do I have knowledge that the ESN or MEID of any Device has been altered or configured for any such purpose at any time.

• Devices with removed or altered serial numbers, IMEI, MEID or other unique numbers, or any alteration to make the device appear to be the original covered device.

• You must include in Your notice of opt-out (a) Your name and address; (b) the date on which You purchased the Product or Accessory; (c) the Product model name; and (d) the IMEI or MEID or Serial Number of the Product or Accessory.
