Medical Substantiation definition
Medical Substantiation. Any request for a leave based upon personal medical reasons shall be accompanied by a physician's statement indicating the nature, anticipated extent, and duration of medical disability. Evidence from a qualified physician indicating the employee's ability to perform all of his/her duties shall be submitted prior to the return of any employee on an unpaid leave for personal medical reasons.
