Medical facilitator definition

Medical facilitator. ’ means a person au- thorized to help complete the application or reen- rollment process on behalf of an applicant or re- cipient under written authorization made by the applicant or recipient. The medical facilitator may help with completing and submitting the appli- cation or reenrollment form, providing necessary information and verifications, and receiving copies of notices or other official communications from the department to the applicant or recipient.   • District Evaluation Advisory Committee means a group created to oversee and guide the planning and implementation of the Board of Education's evaluation policies and procedures as set forth in N.J.A.C. 6A:10-2.3.

  • Esthetician means a person licensed under this chapter to engage in the practice of esthetics.
