  • This medical information may be used by the HCP, Covid Clinic, MedLab, and District for the Intended Purpose as set forth in Section 2 of the Informed Consent.

  • Any information I share through social media or otherwise that set forth herein is shared voluntarily, and I release Covid Clinic, MedLab, the HCP and District from liability under the HIPAA and the CMIA for the information I share.

  • I understand that a physician, duly licensed under applicable state law (the “Health Care Provider” or “HCP”), has been appointed with District’s approval to authorize Covid Clinic or MedLab to perform and report the COVID-19 Testing for and on my behalf, at such times and frequency as the HCP shall direct.

  • District shall indemnify, hold harmless and defend the Covid Clinic Parties from and against all Damages arising from or relating to a third party claim against one (1) or more Covid Clinic Parties based on the District Parties’ negligent act or omission or willful misconduct in the performance of the activities under this Agreement, or the District’s breach of this Agreement, except to the extent that Damages are caused by the acts or omissions of the Covid Clinic and MedLab Parties.

  • Throughout the Term of this Agreement, Covid Clinic shall – and shall cause MedLab 2020 to - procure and maintain, or cause to be maintained, professional liability insurance for itself and its personnel with liability limits of not less than Five Million Dollars ($5,000,000) in the annual aggregate.

  • With the exception of quality assurance testing by Covid Clinic or MedLab using wholly de-identified patient samples in accordance with federal law, no other tests will be performed on my sample.

  • I hereby request and authorize Covid Clinic and MedLab to disclose and transmit the results of my COVID-19 Testing to the HCP.

  • Participating Clinical Laboratory means a Clinical Laboratory which has a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.

  • Non-Participating Clinical Laboratory means a Clinical Laboratory which does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan provide services to you at the time services are rendered.

  • Non-Participating Certified Clinical Nurse Specialist means a Certified Clinical Nurse Specialist who does not have a written agreement with the Claim Administrator or another Blue Cross and/or Blue Shield Plan to provide services to you at the time services are rendered.
