Informed Assent Form definition

Informed Assent Form or “IAF” means an agreement to participate by subjects who are not able to give consent, either because they are minors or because they are legally incompetent.

Related to Informed Assent Form

  • Consent Form means the forms provided to each holder of Pre-Formation Interests to consent to the Formation Transactions and to make such holder’s irrevocable elections with respect to consideration to be received in the Formation Transactions.

  • Letter of Transmittal and Election Form means the letter of transmittal and election form for use by the Corporation Shareholders with respect to the Arrangement;

  • ASBA Form means an application form, whether physical or electronic, used by ASBA Bidders to submit Bids, which will be considered as the application for Allotment in terms of the Red ▇▇▇▇▇▇▇ Prospectus and the Prospectus;

  • Initial Additional Authorized Representative has the meaning assigned to such term in the introductory paragraph of this Agreement.

  • Duly authorized representative shall include any person or persons acting within the limits of his or her authority.
