Information packet definition

Information packet means the information packet that includes a list of service providers who provide "privacy breach response services."

Information packet. The Health Plan shall develop, print, and distribute IDHW accepted information packets to its network of Providers upon Provider enrollment. Provider Information packets shall include, at minimum, the following:
