An extension of insurance benefits up to two months may be approved based on the results of an Individualized Assessment performed under the provision of the ADA, as amended.

Upon Request by the applicant in situations where the applicant is rejected, the applicant has 10-14 days to request an Individualized Assessment from the date they receive the adverse action letter.
Within thirty (30) calendar days of receiving the revised Transportation Policy, Individualized Assessment Tool, and Individualized Assessment Tool Standard Operating Procedures, OCR will review them.
