For providers, a requirement to provide discharge planning services which shall, at a minimum: Coordinate a community-based discharge plan for each Individual served under this Contract beginning at intake.

The person associated with the allegation of abuse, neglect, or exploitation may resume contact with the Individual served under the contract if the investigation results in the person being cleared of all allegations of abuse, neglect, or exploitation.
