  • An independent diagnostic testing facility (IDTF), infusion therapy provider, or ambulatory surgery center (ASC) shall be qualified and duly certified to participate in the CMS program under Title XVIII of the Social Security Act or accredited by a nationally recognized accreditation organization that is approved by state or federal guidelines.
