Housing Navigation Services definition

Housing Navigation Services means the process by which homeless individuals that have entered the Coordinated Entry System (CES), which is the County’s system that connects these individuals to the appropriate services and housing interventions in the community, are provided with ongoing engagement, document collection, and case management services in order to facilitate a match to an appropriate house resource. In the context of CES, outreach workers, case managers, and other providers that service the homeless, may provide housing navigation services. “Supportive Services” means services that may assist homeless participants in the transition from the streets or shelters into permanent or permanent supportive housing, and that assist persons with living successfully in housing. To the extent that services are not covered under the CalOptima Medi-Cal program, Housing Navigation and Supportive Services may include, but are not necessarily limited to the following:
Housing Navigation Services means supportive services to Client in order to gain 11 residential housing placement.

  • The OPERATOR will provide the COUNTY with an Employment and Housing Navigation Services Policy in accordance with the Phase I MOPSP and will include policies and procedures for documentation of Employment and Housing Navigation Services, Data Security, confidentiality, client privacy and consent, tracking client progress and participation, resources and referrals given, support rendered, and any infractions the client may accrue.

  • Housing Navigation Services shall focus on the marketing aspects of housing navigation, including locating available units, recruiting new landlords, negotiating with landlords on incentives, risk mitigation funds, and any flexibility in lease terms and conditions.

  • Housing Navigation Services shall provide transportation to participants to support the housing search process, attend housing meetings, viewing and/or interviews.

  • Housing Navigation Services shall not be limited to these homeless service system interventions but also consider other permanent housing options that are affordable and sustainable for the individual, including but not limited to shared housing, room for rent, family reunification, etc.
