HealthWorks definition
HealthWorks means a comprehensive system of health care developed by DCFS for all Illinois children and youth in ▇▇▇▇▇▇ care that ensures they have access to quality health care, routine health care and special health care that meets their identified health care needs and provides documentation of health needs and health care information that is readily accessible to caregivers, other healthcare providers and DCFS. HealthWorks provides access to and referral for primary health care physicians, initial health screenings, comprehensive health evaluations, well-child examinations and immunizations.
HealthWorks means a statewide medical program established to provide a primary care physician and medical case management for Department involved youth under age six or pregnant and/or parenting.
