Health Action Plan (HAP) definition

Health Action Plan (HAP) means an enrollee-prioritized plan identifying what the enrollee plans to do to improve his or her health.
Health Action Plan (HAP) is a Beneficiary prioritized plan identifying what the beneficiary and others plan to do to improve his or her health.

Examples of Health Action Plan (HAP) in a sentence

  • Care management services include state approved screens and development of a person-centered Health Action Plan (HAP).

  • The Health Action Plan (HAP) should contain at least one Enrollee-developed and prioritized goal; identify what actions the Enrollee is taking to achieve the goal(s); and includes the actions of the Intensive Care Coordinator, including use of health care or community resources and services that support the Enrollee’s Health Action Plan.

  • For Level Three Enrollees, the ICP should incorporate information from his/her Health Action Plan (HAP), discussed further below.
