Form 4C means the Exchange’s Form 4C – Corporate Placee Registration Form to be completed by all Subscribers that are not individuals unless such Subscriber has previously filed a Form 4C with the Exchange and it is still current;

  • State whether the Subscriber has a current Form 4C on file with the TSX Venture Exchange (if not an individual): Yes [ ] No [ ] 4.

  • The Subscriber agrees to deliver to Agent and such other person as Agent may direct, not later than 4:00 p.m. (Houston, Texas time) on the day that is at least three business days before the Closing Date: (a) this duly completed and executed Subscription Agreement; (b) Exhibit 1 and Exhibit 3 duly completed; and (c) if the Subscriber is not an individual, one completed and executed TSX Venture Exchange Form 4C, Corporate Placee Registration Form attached hereto as Exhibit 4.

  • The Investor will, or will cause others to, as applicable, execute, deliver, file and otherwise assist the Company in filing on a timely basis, such reports, undertakings and other documents required by applicable Laws in connection with the transactions contemplated hereunder, including, if applicable, a Form 4C – Corporate Placee Registration Form for the Investor and a personal information form or declaration form for the Investor’s Nominees to the Board.

  • If you are not an individual, you either [CHECK APPROPRIATE ITEM]: HAVE PREVIOUSLY FILED with the TSX Exchange a Form 4C, Corporate Placee Registration Form, and there has been no change to any of the information in the Form 4C previously filed; or HEREBY DELIVER to the Corporation a completed Form 4C in the form attached as Exhibit D for filing with the TSX Exchange.

  • The Subscriber, IF NOT AN INDIVIDUAL, either [CHECK The Subscriber either [CHECK APPROPRIATE ITEM]: has previously filed with the Exchange a Form 4C, Corporate Placee owns directly or indirectly, or exercises control or direction over, NO the Exchange up to the date of this Subscription Agreement; or (excluding the securities subscribed for herein); or attached hereto for filing with the Exchange.

  • The Bidders shall submit Form 4C, Section IV in support of this criteria.

  • CONTRACTOR’s payments shall be as provided in the Price Sheet, Required Form 4-C, for Assessment and Intervention Services and as supported by Required Form 4-F, Line Item Budget, and the CONTRACTOR shall be paid only for the tasks, deliverables, goods, services, and other work approved in writing by the COUNTY.
