  • This description shall be made available to FIDA Plan Participants upon request.

  • The FIDA Plan, at all times, shall be in compliance with all financial requirements of 10 NYCRR Part 98 and 42 C.F.R. § 438.116.

  • CMS will also submit a file to NYSDOH identifying individuals who have elected to disenroll from a FIDA Plan, Opt Out of Passive Enrollment, or have enrolled in or have selected another type of available Medicare coverage that is not a FIDA Plan.
