  • I release FBISD from any liability for the quality, timeliness, and outcome of any such medical care received and for any expenses incurred.

  • I authorize FBISD to arrange for medical care and treatment for the Student if the Student becomes ill or incapacitated, without incurring any liability.

  • FBISD and its authorized representatives shall take any action deemed necessary to protect the Student's mental and/or physical health, at the Parent/Legal Guardian's expense.
