FAIR BILLING. The standard, usual and customary rate charged by the Contractor, which should be consistent with the amounts charged to any other party for similar services. HOSPITAL SERVICES: Inpatient or Outpatient Services necessary to correct or substantially modify, within a reasonable period, a condition that is stable or slowly progressive.
