Eye Pathology definition

Eye Pathology. DVR has the primary responsibility of serving any eligible individual who has one-eye pathology or other disabilities which result in a substantial impediment to employment. Such an individual may have other disabilities which are in addition to the visual impairment, such as other physical and/or mental disabilities. DVR has the responsibility of providing services needed to address those additional disabilities.

Examples of Eye Pathology in a sentence

  • After 1 per year: Not covered: Member pays 100% of all charges Contact Lenses (in lieu of eyeglasses): Member pays 50% coinsurance Contact Lenses or Framed Lenses for Eye Pathology: No charge; Member pays nothing Reduction of a fracture or dislocation of the jaw or facial bones; excision of tumors or non-dental cysts of the jaw, cheeks, lips, tongue, gums, roof, and floor of the mouth; and incision of salivary glands and ducts.

  • Frames and Lenses: Member pays nothing, limited to an Allowance of $150 every 12 months After Allowance: Not covered; Member pays 100% of all charges After Allowance: Not covered; Member pays 100% of all charges Contact Lenses or Framed Lenses for Eye Pathology: No charge; Member pays nothing Members to age 19: Eyeglass frames, lenses (any type), lens options such as tinting, or prescription contact lenses, contact lens evaluations and examinations associated with their fitting.

  • Routine Exams: Member pays $25 Copayment Exams for Eye Pathology: Member pays $25 Copayment Members age 19 and over: Eyeglass frames, lenses (any type), lens options such as tinting, or prescription contact lenses, contact lens evaluations and examinations associated with their fitting.

  • The benefit period begins on January 1 and continues through the end of Frames and Lenses: Member pays nothing, limited to an Allowance of $100 every 24 months After Allowance: Not covered; Member pays 100% of all charges Contact Lenses or Framed Lenses for Eye Pathology: No charge; Member pays nothing Frames and Lenses (in lieu of contact lenses): No charge; Member pays nothing for up to 1 per year the calendar year.

  • Frames and Lenses: Not covered; Member pays 100% of all charges Contact Lenses or framed lenses for Eye Pathology: After Deductible, Member pays nothing Members to age 19 – Routine eye examinations and refractions, coverage is limited to one per calendar year.
