Examination Data definition

Examination Data means any data provided by an end user to Service Provider or data that an end user or an end user’s legal guardian has consented for Service Provider to retain and use and any and all data collected by Service Provider during registration and administration of an examination of any end user.

Examples of Examination Data in a sentence

  • Healthcare Provider Data – Maternity, Newborn and Physical Examination Data, Hearing Screening Data, Vaccination data, etc n/a 4.

  • Section 2: Service Provider agrees that all information regarding TJJD and/or its youth that is gathered, produced, or otherwise derived from this contract, excluding Examination Data, shall remain confidential and subject to release only by permission of TJJD.

  • Section 5: Notwithstanding anything contained herein, Examination Data shall not be considered student records.

  • If the participant has already visited a pediatrician and completed the exams, they will have access to different tabs as follows: Tab 1 – Medical Examination Data: Displays the data and measurements captured by the pediatrician (e.g., height, weight).
