Entity Location(s) definition

Entity Location(s) means those individual Entity locations listed on the HRSA web-site pursuant to an executed enrollment or registration form which are authorized as Covered Entities to contract with a licensed pharmacy to manage and dispense Covered Drugs and locations or sites that are considered to be part of the Entity in accordance w1th applicable 340B Pharmacy Program standards as determined by Entity.

Examples of Entity Location(s) in a sentence

  • Either party may suspend this Agreement or any portion thereof (including but not limited to, any 340B Drugs, Pharmacy Locations, Covered Entity Locations, and eligible prescribers) at any time either: (i) without cause upon thirty (30) calendar days’ prior written notice to the other party; or (ii) for material breach immediately upon written notice to the other party.

  • Covered Entity and Walgreens will establish a mutually agreed upon implementation schedule, which may include implementing 340B Drugs, Pharmacy Locations, Covered Entity Locations, and eligible prescribers on a rolling basis.
