ENCOUNTER FORM definition
ENCOUNTER FORM means a HCFA 1500 or a form supplied to GROUP PROVIDER by COMPANY upon which the GROUP PROVIDER enters all information relative to Enrollee's use of capitated Billaboves and Primary Care Covered Services.
ENCOUNTER FORM means a HCFA 1500 or a form supplied to ANCILLARY PROVIDER by COMPANY upon which the ANCILLARY PROVIDER enters all information relative to Enrollee's use of Covered Services.
