Case Management Linkage Brokerage means a process of identification, assessment of need, 13 planning, coordination and linking, monitoring and continuous evaluation of Clients and of available resources and advocacy through a process of casework activities in order to achieve the best possible 14 resolution to individual needs in the most effective way possible. This includes supportive assistance to 15 the Client in the assessment, determination of need and securing of adequate and appropriate living arrangements.
