I understand that I am to apply sunscreen to my child before dropping him/her off to attend an ECDS Summer Campz program.
I authorize my child attending an ECDS summer program to participate in visits to various San Diego and Southern California destinations, including but not limited to theme parks, museums, science centers, local beaches, etc.
Data systems that may be eligible for ECDS reporting include, but are not limited to, administrative claims, clinical registries, health information exchanges, immunization information systems, disease/case management systems and electronic health records.

Permission will not be required, unless ECDS specifically requests it, where Author has made a reasonable and good faith determination of fair use, after careful consideration of each given use of third-party materials that Author wishes to make.
I understand that if my child leaves the ECDS Campzus to attend an off-Campzus field trip, he/she will be transported in a safe, seat-belted vehicle driven by a staff member, a parent-driven carpool, or a professionally driven charter bus.
