DM and PM Services definition
DM and PM Services means a multi-disciplinary approach, based on evidence-based guidelines, to the delivery of healthcare services that (i) assists individuals with self managing such individuals' health and wellness, (ii) proactively identifies individuals at risk for or with a disease and empowers such individual to become a part of such individual's health care team, (iii) support the individual/physician relationship, and/or (vi) continuously evaluate individuals' clinical, humanistic, and economic outcomes with the goal of improving such individuals' health.
