Direct provider definition

Direct provider means a health care professional licensed, accredited, registered, or certified to perform specified primary care health services consistent with the law of this state. “Direct provider” includes an individual health care professional or other legal health care entity alone or with other health care professionals professionally associated with the individual health care professional or other legal health care entity.
