Direct Debit Authorisation Form definition

Direct Debit Authorisation Form means the separate authorization in the AFFINBANK’s prescribe form, duly completed by the Cardmember and the Programme Merchant at the time of purchase for submission by the Programme Merchant to AFFINBANK, whereby the Cardmember agrees to observe and be bound by these Terms and Conditions. The details and particulars provided by the Cardmember in the Direct Debit Authorisation Form shall be deemed final and irrevocable.
