The Contractor and the DHS Designee will mutually determine the targeted admission date and mutually confirm the actual admission date after receiving confirmation of the Department’s final approval.
The Contractor shall coordinate and participate in a minimum of one Transition Care Conference prior to the targeted admission date with the DHS Designee of both the referring and receiving Department or AAA branch and a representative of the provider(s) currently providing services to the Individual (as applicable).

No Individual(s) served under this Contract may be discharged from the facility without the prior review and approval by DHS Designee and the Individual’s SPT.
Contractor shall notify DHS Designee and DHS Contract Administrator of an unexpected and immediate absence of the Individual from the program.
Contractor shall notify the DHS Contract Administrator and DHS Designee in writing of their intent to issue an involuntary move-out notice.
Contractor will submit health and safety trending reports and annual staff training reports upon request by DHS Designee or DHS Contract Administrator.
